In many cases, we can have a caregiver ready the same day your loved one comes home. We work directly with hospital discharge planners to coordinate timing. For planned surgeries, we recommend scheduling care before the procedure so everything is arranged when you arrive home.
Traditional health insurance and Medicare do not cover non-medical home care services like ours. However, long-term care insurance often does cover post-surgical home care. We can help you check your policy. For medical home care (nursing, physical therapy), a home health agency can bill Medicare directly. Many families use both services together during recovery.
Our caregivers can encourage and assist with prescribed physical therapy exercises — things like walking practice, range-of-motion movements, and balance activities that your therapist has demonstrated. We do not design exercise programs (that requires a licensed physical therapist), but we help ensure exercises are performed safely and consistently between therapy sessions.
Roughly one in five Medicare patients is readmitted within 30 days, and most of those readmissions are preventable. Our recovery shifts focus on the specific high-risk windows: medication accuracy after discharge, fall prevention during the first two weeks, hydration and nutrition, watching incision sites for early infection signs, and making sure the patient actually attends every follow-up appointment. Anything concerning gets a same-day call to the family and (with consent) the discharging team.
Yes — and this is one of the highest-value parts of having an agency caregiver during recovery. We share a written daily log with the home-health nurse, physical therapist, and occupational therapist (with the family's consent), so the clinical team sees real day-to-day progress instead of a snapshot once a week. This shortens the recovery curve and catches setbacks early.
