The 100-day SNF benefit is one of the most misunderstood pieces of Medicare. It only applies when the patient had a qualifying inpatient hospital stay of at least 3 nights (observation status does not count) and is admitted to a Medicare-certified SNF within 30 days for a related skilled need — typically rehab after stroke, hip fracture, joint replacement, or major cardiac event.

Coverage is not guaranteed for the full 100 days. Days 1–20 are fully covered. Days 21–100 carry a daily copay ($204/day in 2025 — verify the current-year amount). And the SNF must certify that the patient is still making measurable progress on skilled goals.
